I agree with A, B.  

For C, in addition to what was stated, I actually think it is appropriate to specifically require [x] amt of time spent active in the business meeting.

For D, if the goal is to help us identify an individual by additionally listing their email address, then I think stating so simplifies things:   
Name _______ (real name, no pseudonyms or screen names)
"ASP Registered" Email Address _________

I also liked the recommendation:  each trusted servant should have a copy of the service manual.
